In this study we evaluated 12 HIV+ patients that received kidney transplantation at Montefiore Medical Center from 2010-2015. The objective of this study was to recognize the population of complement binding donor specific antibodies using C1q methodology (One Lambda). Our target cohort group where special cases that may be not fully understood by the clinical team due to HIV+ status. C1q testing may provide better understanding of complement binding activity post-transplantation and clinicians will be able to deliver greater quality medical care. All patients had a negative CDC crossmatch upon transplantation. Out of 12 patients, 6 had positive Flow crossmatch upon transplantation. Many HIV+ patients develop HIV nephropathy and require kidney transplantation (2). HIV binds to the surface proteins that bind antigens on host cells. HIV protein gp120 binds a combination of CD4 and CXCR4 on T-cells. Cells are killed by direct cytopathic effect, which is an anti-viral response or by transformation of host cells via oncogene expression (3). HIV effects immune system and may interfere with interpreting correct results or placing compatible organs (2). Crossmatching an HIV+ patient with potential donors poses serious challenges due to HIV induced B- cell deregulation and presence of auto-antibodies. HIV+ patients may have false positive test results that may be difficult to interpret or find compatible organs. HIV+ cases can create confusion, therefore supplementary testing is needed to better assess suitability of the organ or graft tolerance (2, 3). I have collected reports of Mrs. Luigi, 65 whom I was suppose to care that day. She was under incubation and was sedated after hip surgery. She was surrounded with equipments and tower of I.V. infusions pumps. I have finished quick assessment of her ensuing not to miss any details though I was busy. I found her stable. Next to it I was showered by lots of questions by her anxious family. I brought them to the bedside and considering them as my own patient I have answered all the questions and gave sufficient and relevant information related to Luigi. I reassured them while I continue to perform my assessment and care. To cater for their emotional and spiritual needs I have asked a chaplain to visit the family members. My assessment was crucial for the multidisciplinary team and the team was waiting for my feedback on Luigi.
